Our Partial Hospitalization Program (PHP) is a 30-90 day intensive track program combining structured housing with clinical services, case management support, and life-skills development to provide comprehensive, wraparound care.
We serve adults struggling with co-occurring substance use disorder, medical care, and mental health needs. They must meet criteria for program compliance and are often transitioning from inpatient care or TSZ program. Designed for individuals who benefit from structured support.
Medicaid must be active to get funding.
