Services
- Therapeutic Support: Daily individual and group therapy is provided to address mental health conditions and substance use recovery. Clients engage in evidence-based practices such as CBT and trauma-informed care, as well as holistic approaches like mindfulness and wellness activities.
- Biopsychosocial Approach: Each client receives a comprehensive biopsychosocial assessment that guides a personalized treatment plan, ensuring all aspects of mental, physical, and social health are addressed.
- Life Skills Training: To aid in the transition to independent living, clients participate in life skills training, such as budgeting, communication skills, and self-care routines.
- Intensive Case Management: Each client will work with a case manager to coordinate care among multiple providers in the community. Focusing on long term needs such as: housing, employment, transportation, assistance to access healthcare and other social services.
- Medication Management: Involves the assessment, prescription, monitoring, and adjustment of medications for patients to ensure they receive the appropriate treatment while minimizing risks of side effects, interactions, or non-adherence.
